Covenant Achievements
Throughout Dark Souls, there are various covenants you can join, but to get the achievement, you only need to be offered to join thus the description of "discover". However, you will have to join some later on for the other achievements.
Covenant: Way of White - Discover Way of White covenant
You can join this covenant at the start of the game by talking to Petrus when you reach Lordran. He's the fat knight hanging around Firelink Shrine.
Covenant: Gravelord Servant - Discover Gravelord Servant covenant
During your trip in the catacombs, you should come across a narrow hallway filled with stone coffins along with a Titanite Demon. One of the coffins should be protruding out and giving you the option to get inside it. It'll take you to Gravelord Nito who will offer you to join.
Covenant: Chaos Servant - Discover Chaos Servant covenant
After beating Quelaag in Blighttown, proceed through the boss area and up the stairs she came down from. Ring the bell and head downstairs. Once down, you should see an cob-webbed exit. To the right is an illusory wall that you can break to unveil the Chaos Servant covenant. Respond Yes to the Eingyi, the weird egg head guy, and he'll let you speak to the Daughter of Chaos to join the covenant.
Covenant: Princess's Guard - Discover Princess's Guard covenant
After beating Ornstein and Smough, there will be two elevators in the back that will bring you up to the Chamber of the Princess. Warning: Amazing chest ahead and Need holding with both hands. You'll know what I mean, Gwynevere will offer you to join the Princess's Guard after receiving the Lordvessel.
Covenant: Blade of the Darkmoon - Discover Blade of the Darkmoon covenant
One of the more hidden covenant, inside the catacombs is the Darkmoon Seance Ring. This is a must to join. After getting the ring, head to Anor Londo and go to the manual rotating staircase. Spin it counter-clockwise and head downstairs. Make sure you're wearing the ring and rest at the bonfire. The statue will disappear and you can proceed to the carpet which will offer you to join the covenant.
Covenant: Warrior of Sunlight - Discover Warrior of Sunlight covenant
Either defeat the Hellkite Dragon on the bridge in Undead Parish/Burg or run through under him to get to the bonfire. This is the bonfire of the Sunlight Altar. Pray at the altar to the right to be offered to join.
Covenant: Forest Hunter - Discover Forest Hunter covenant
While in the Royal Woods behind the Crest of Astorias or through the Darkroot Basin, there is a bridge connecting two cliffs and a building. In one of the buildings is Alvina, a cat, in the window sill. Alvina is the head of the forest covenant and will offer you to join.
Covenant: Darkwraith - Discover Darkwraith covenant
After obtaining the Lordvessel, DO NOT speak to Frampt in Firelink Shrine. Instead, make your way to New Londo Ruins and defeat Four Kings. Darkstalker Kaathe will appear and warp you to the Firelink Altar (not Shrine). Place the Lordvessel down and warp back out to the Abyss using Kaathe. He will then offer you to join.
Covenant: Path of the Dragon - Discover Path of the Dragon covenant
Hidden in Blighttown is a large branch on the other side of the swamp to Quelaag's Domain is a giant branch. Climb up the large branch and break down two illusory walls which will take you to The Great Hollow. Climb all the way down to reach Ash Lake. Reach the end of Ash Lake to meet the Everlasting Dragon who will offer you to join the Dragon covenant.

Light your first bonfire! Yipee!
Estus Flask - Acquire estus flask
After being jewed by a bowling ball, turn around to a now broken wall and talk to the knight who will give you the Estus Flask.
Reach Lordran - Arrive in Lordran
After defeating the likes of the Asylum Demon, pass through the doors and to the edge of the cliff to be kidnapped by a raven who takes you to Lordran.
Ring the Bell (Undead Church) - Ring Bell of Awakening at Undead Church
After killing the Belfry Gargoyles, ring the bell up the stairs and ladders.
Ring the Bell (Quelaag's Domain) - Ring Bell of Awakening at Quelaag's Domain
After killing Quelaag, proceed through her domain and ring the bell.
Rite of Kindling - Acquire the Rite of Kindling
In the Catacombs, defeat the Pinwheel boss to receive the Rite of Kindling
Art of Abysswalking - Acquire the Art of Abysswalking
Inside the Royal Woods, defeat the Great Grey Wolf Sif to receive the ring, Covenant of Artorias which will allow you to cross into the Abyss safely.
Reach Anor Londo - Arrive in Anor Londo
After beating the Iron Golem at the top of Sen's Fortress, examine the gold ring so bat demon can kidnap you and take you to Anor Londo.
Lordvessel - Acquire the Lordvessel
After beating Ornstein and Smough, go up the elevator and talk to Gwynevere.
To Link the Fire - Reach "To Link the Fire" ending
After beating Gwyn, Lord of Cinders, rest at the bonfire to proceed to NG+.
Dark Lord - Reach "Dark Lord" ending
After beating Gwyn, Lord of Cinders, walk away from the bonfire and out the door to proceed to NG+.

Defeat Gravelord Nito - Defeat the Soul Lord Gravelord Nito
Defeat Bed of Chaos - Defeat the Soul Lord Bed of Chaos
Defeat the Four Kings - Defeat the Four Kings, inheritors of souls

Knight's Honor Achievement
Pretty much, you have to have all these weapons on a single character in order for this achievement to unlock. To get this achievement, you must go through NG, NG+, and half of NG++ due to the fact that you need Sif's soul three times. If you intend to get all achievements in one gameplay, then you will also need Gwyn's soul which is obtained as you start NG+ and so forth which you need two of for a weapon upgrade listed below as well as a miracle.
To upgrade weapons to +10, you need the Large Ember given to Andre of Astora in the Undead Parish to ascend a +5 weapon to +6 and then continue reinforcing.
Boss Soul Upgrades
Weapons that are upgraded with boss souls can only be done by the Giant Blacksmith in Anor Londo
Chaos Blade
Upgrade a +10 Katana with the Soul of Quelaag (Blighttown)
Katanas may be found, purchased from Shiva of the East or kill the male Undead Merchant

To upgrade a weapon past the default +5, or a weapon of different attributes, you need to obtain the corresponding ember as well as giving that ember to the correct blacksmith. There are a total of four blacksmiths in the entire game. Every achievement requires a slab of a certain color.
